Insurance & Payment Policy
Important: Insurance coverage and reimbursement are not guaranteed. Benefits depend on the client’s insurance plan, eligibility, testing type, carrier rules, deductibles, co-payments, co-insurance, medical necessity, authorization requirements, and employer program requirements.
Insurance Coverage
IronClad Workplace Testing accepts most major insurance plans for eligible testing services. Clients are responsible for confirming benefits with their insurance carrier before scheduling.
$30 Mobile Collection Professional Fee
A $30.00 Mobile Collection Professional Fee may be required before arrival to secure the appointment and dispatch a testing professional to the requested location.
This fee is separate from laboratory billing, insurance reimbursement, employer program billing, or any additional service charges. This fee may not be reimbursable by insurance.
Appointment Confirmation
- Appointment request must be received.
- Required information must be provided.
- Applicable fees must be paid before arrival.
- IronClad Workplace Testing must issue confirmation.
